Every Organization that is an Endowment Partner of the Community Foundation should designate a primary contact who will receive written communications, including emails, from the Foundation. As a part of our service to our Endowment Partners, we strive to keep you informed and up to date with information that could help you in your efforts to build endowment. We rely on you to give us the proper contact information, and to keep it up to date. You can name other contacts within your Organization who also can receive communications from the Foundation, but we appreciate your using this form to designate the primary contact.
Similarly, only authorized persons are able to request distributions from each Organization's endowment fund at the Foundation. As board members change, and officers rotate to new positions or step down from your board, you will need this form to keep the Foundation apprised of the authorized persons from your Organization's board.
Please complete the Fund Authorization and Contact Form and fax it or email it back to the Foundation whenever there is a change in the information it contains.
